Whether you are a new or established patient, it is always best to have these items:

• Photo ID
• Insurance card(s)
• A form of payment, such as check, cash or credit cards to cover your co-pay or deductible
• Your most up-to-date lab information or medical images
• Your list of current medications including over-the-counter and prescription medications
• A list of questions you have for your doctor

To ensure you don’t feel rushed, we suggest checking in at your doctor’s office 15 minutes before your scheduled appointment. This extra time gives you a chance to fill out any new patient information, review your questions and decide what you want to talk about during your appointment.
